The County of San Diego Aging and Independence Services (AIS) is soliciting proposals to operate the Home Safe Program, a statewide initiative funded by the California Department of Social Services (CDSS) to address housing instability among older adults (age 60+) and dependent adults with disabilities. The selected contractor will provide intensive case management and wraparound services — including housing search assistance, emergency placements, linkage to Adult Protective Services (APS), medical and behavioral health referrals, benefits enrollment, and in-home safety planning — to APS-referred clients who are homeless or at imminent risk. The contractor must coordinate closely with AIS APS staff and local housing and health systems. This contract replaces expiring Contract #567352. • Demonstrated experience with APS-involved older adult or adult disability populations • Licensed Social Workers (LCSW) or clinical supervisors overseeing all case management • Housing placement capability: established relationships with landlords, shelter providers, and SNF facilities • Home Safe Program compliance: CDSS reporting in the statewide data system • 24-hour response for APS emergency housing placements • Benefits enrollment assistance: Medi-Cal, SSI/SSDI, CalFresh, IHSS • Elder/dependent adult abuse identification and mandated reporter training for all staff • General Liability Insurance: $1,000,000 per occurrence • Professional Liability: $1,000,000 per occurrence • Background clearances for all staff working with vulnerable adults • Monthly CDSS program data submissions and quarterly outcome reports to AIS
Eligible applicants include licensed social service agencies, nonprofits, and community-based organizations with demonstrated experience providing intensive case management, housing navigation, and Adult Protective Services-related services to older adults and/or adults with disabilities. Organizations must be familiar with the California Home Safe Program requirements and CDSS reporting standards.
